In order to reach the Rauh Ruins in the top-left corner of the Realm of Shadow — and also grab its Map — you'll need to start off in the centre of the Scadu Altus, the Site of Grace in Moorth Ruins to be precise. Fast travel there and open up your map. Just northeast of you is a small pool of water in a triangular shape. Please see the Beacon in the image for an additional guide:

Go here and you'll find a cave with a Scadutree Fragment in front of it. Go inside and follow the linear path through until you come to a golden, glowing tree. To the right of it is a path that'll drop you down into a swamp. Follow the path to your left underneath the rock, and on the other side, text will appear revealing you've reached Rauh Base. A Site of Grace for the Ancient Ruins Base will then be directly ahead of you.

In order to get the Map for the Rauh Ruins, continue down the hill past the ruins, and you should notice two giant enemies with a bow and arrow and an axe. Feel free to either take them down or run past, then just beyond them you should notice the remnants of buildings sticking out of the ground. Next to them is the Rauh Ruins Map, revealing the area on your personal map.
