After about a year of owning the system, I'd say my top five are, in no particular order:
Dragon Quest Builders
Final Fantasy XV
The Witcher 3: Wild Hunt
Gravity Rush Remastered
Uncharted 4: A Thief's End
Honorable mentions go to God Eater Resurrection/2 (as they're basically the same game), Metal Gear Solid V, and Dark Souls III.
Horizon will probably make its way into my top 5, but I don't want to kick The Witness out either... Tough decisions, I'll get back to this thread when I'm done with HZD.
Old List
1. Overwatch
2. Modern Ware Remastered
3. Black Ops 3
4. Fallout 4
5. Lego Dimensions
New List
1. Diablo III UEE
2. Overwatch
3. For Honor
4. Modern Warfare Remastered
5. CoD: Black Ops III
Yeah with the introduction of Seasons Diablo III has risen to the top again for me. Newcomer For Honor has made the list while staples Overwatch and Black Ops 3 remain along with Modern Warfare. Next month I am sure will change with The Disney Afternoon Collection on there somewhere since it had some of my favorite games of all time include in it.

1 - Ratchet and Clank
2 - Just Cause 3
3 - ADIOS
4 - The Binding of Isaac: Afterbirth (Temporarily, until Switch release)
5 - Mighty No. 9 (It's better than Far Cry 4 okay?)
Now Playing: Mario & Luigi Brothership, Sonic x Shadow Generations
Now Streaming: The Legend of Zelda: Echoes of Wisdom
5. Inside
4. The Last of Us Remastered
3. GTA V
2. Horizon: Zero Dawn
1. Uncharted 4
Including TLOU and GTA 5 is a little odd seeing as they're just upgrades of a previous gen but they're still 2 of my favourite games on the system so feel like I have to include them.
1. Uncharted 4
2. Bloodborne
3. Nioh
4. Persona 5
5. Horizon Zero Dawn
Uncharted 4 & Bloodborne are in a league of their own. I don't think anything will top them. Only 30 hours into Persona 5, but it's going to take something special to topple Nioh. You can't deny that pure gameplay experience.
Not to be a fanboy , but this is also why exclusives are important imo. Tailor made to provide the best experience possible.

@Ralizah I couldnt get into Velocity 2x at all a few years ago. It was one of the first games I played on the PS4 igc so maybe I didn't give it a fair chance.

@themcnoisy It's fantastic. Already platinumed Velocity Ultra, so I figured I'd work on this one. It's twitchy, perfectionist shmup action at its absolute best. I think, like Yoshi's Island, it's a game that one grows to appreciate far more if one strives to actually 100% it. Learning to master successive levels is a wonderful experience.

Picking 5 is really hard. Do I pick what I was most hyped about at the time or the things with the most lasting power? HZD was an amazing game, but I'm unlikely to replay it or think about it too much after its done. Diablo 3 is an ancient game from last gen, but apparently has amazing replay value. Hard to say which to rate higher. Souls games have both, so I guess there's that
@Ralizah@themcnoisy Velocity 2X is amazing, really fun game. I should go back and finish the plat but perfecting the last 3 levels was too hard last time
My List, not in order:
Bloodborne
Dark Souls 3
Diablo III UEE
Resogun
Borderlands: The handsome collection
Honorable mentions:
Ratchet & Clank
Velocity 2X
Nioh (I find myself wanting to replay DS3 more than this, surprisingly).
@Mega-Gazz I'm expecting the last few levels to be nightmarish. It took me days to get a perfect rating on level 50 in Velocity Ultra.
@KratosMD I agree. Exclusives give a console character. More than that, though, they often provide experiences other systems simply can't provide because they're designed to the specifications of a specific platform. Imagine how much less of a technical marvel a multiplat Uncharted would have been. Imagine trying to play Tokyo Mirage Sessions without the added functionality of the GamePad's second screen. Games designed for a specific platform are able to take advantage of its unique features. This is why, contrary to what a lot of people say, Nintendo should never go third party.
Horizon Zero Dawn
Persona 5
The Witcher 3
Uncharted 4
Hmmm...those are a clear top 4, probably in that order. After that I am left with a bunch of honorable mentions that are great but don't quite reach the heights of the other 4.
Ratchet and Clank, Infamous Second Son (This game graphically shows age and the side quests could have been better, but I LOVED it), MGSV, Final Fantasy XV/World of Final Fantasy, Shovel Knight, Fallout 4
