There are 8 endings in Silent Hill 2, of which 6 return from the original game and a further 2 are entirely new for the PS5, PC remake. How the game handles unlocking these endings isn't like most other games, however. Rather than obvious decisions you make throughout your playthrough, Silent Hill 2 bases what ending you get on how you play the title, and in the case of New Game+, what secret items you get. At no point is there an on-screen choice for you to make; how the endings are handled is a lot more vague.

Luckily, we can pull on over 2 decades of experience with the PS2 game, since the requirements for the original endings in the PS5, PC remake mostly remain the same. You'll be judged on how much you heal James, how you handle Maria, and more. Below you'll find how to get all Silent Hill 2 endings. There's a Trophy for unlocking each original ending.

How to Get the Leave Ending in Silent Hill 2

Due to its requirements, the Leave ending is what most first-time players will get on their first playthrough. It requires you to keep James healed up as much as possible, which would be a natural need for a newcomer, and to not check in with Maria when you're in Brookhaven Hospital and the Labyrinth. You also cannot interact with Angela's Knife in your inventory for too long, which a new player would never know they need to do in the first place. Essentially, this ending is all about showing your commitment to your wife Mary throughout a playthrough rather than taking an interest in Maria.

To get the Leave ending, fulfil the following requirements:

  • Heal yourself as much as possible
  • Pick the Snake Coin as the final solution to the Coin Puzzle
  • Listen to the whole sickbed conversation between James and Mary in the hallway before the final boss

Do Not...

  • Examine Mary's Letter, Mary's Photo, or the Handerkerchief in your inventory
  • Check on Maria in Brookhaven Hospital and the Labyrinth
  • Interact with Angela's Knife in your inventory

The Maria ending is essentially the opposite of the Leave ending, where you prioritise Maria over Mary. When she accompanies you during gameplay, you need to keep her safe as much as possible. Avoid allowing her to take damage, and protect her from enemies. Then, when you are able to check on her in Brookhaven Hospital and the Labyrinth, you must do so. You need to avoid interacting with Mary's Photo and her Letter in your inventory to really hammer the point home, don't listen to the entire sickbed conversation at the end of the game.

For the In Water ending, you basically need to let James get beat up and rarely help him. Instead of healing him after being attacked, you must leave James in a damaged state where there's red blood on the screen as much as you can. Then, you'll want to interact with Angela's Knife in your inventory often after getting it in Blue Creek Apartments, but avoid looking at Mary's Photo and her Letter.

To get the In Water ending, fulfil the following requirements:

  • Pick the Man Coin as the final solution to the Coin Puzzle
  • Interact with Angela's Knife in your inventory multiple times
  • Listen to the whole sickbed conversation between James and Mary in the hallway before the final boss

How to Get the Rebirth Ending in Silent Hill 2

The Rebirth ending is only possible on a New Game+ playthrough, as you need to find and collect 4 new items spread throughout the game that are only unlocked in this second run. They are, in the order you find them in the game:

You'll find these 4 items in different locations throughout the game on a New Game+ run. Once you have them, you simply have to complete the game again and you'll get the Rebirth ending automatically.

How to Get the Dog Ending in Silent Hill 2

The Dog ending is one of 2 more comical conclusions to Silent Hill 2, and is another that can only be unlocked in a New Game+ playthrough. In order to get it, you must find the following items:

Once you have them, you can combine them in your inventory and save the Dog Key until you reach the Lakeview Hotel. You can use it to enter the Observation Room in Lakeview Hotel, which is on the same corridor as Room 312. This ends the game in a very funny way once you head inside.

How to Get the UFO Ending in Silent Hill 2

The second humorous ending found in New Game+, you need to find a new item called the Blue Gem and then use it in 3 specific places in the game. They are:

  • The rooftop of Saul Street Apartments in East South Vale
  • On the pier at Rosewater Park before meeting Maria
  • On the pier after rowing across Toluca Lake to the Lakeview Hotel

How to Get the Bliss Ending in Silent Hill 2

The Bliss ending, which is the first of two entirely new conclusions, can only be gotten in New Game+. When you're exploring West South Vale, head to Pete's Bowl-O-Rama to find a new Safe you can open to get the Rusted Key. The Safe code is 1887. You now need to progress to Brookhaven Hospital, where in the gazebo in the Garden, you'll find another new chest. Open it using the Rusted Key to acquire a drink called White Claudia.

Now, once you reach Room 312 in Lakeview Hotel, use the White Claudia from your inventory and then play James' Tape. This will trigger the Bliss ending.

How to Get the Stillness Ending in Silent Hill 2

The second brand-new ending found in the PS5 remake, the Stillness ending is only available in New Game+. Your first playthrough must have ended in the In Water ending for this to work. At the start of the game, there'll be a green car you can break the windows of using the Chainsaw. This car is on Wiltse Road and is located before the first enemy encounter, beyond the turning you'd take in between the houses. In the car, you'll find the Key of Sorrow.

Now, you need to progress all the way to the end of the game, to when you're in the Otherworld version of Lakeview Hotel. This is after you watch the cutscene in Room 312. Go to the Manager's Office to find a new Safe. The Key of Sorrow can be used on the lock, and the code 314 works on the combination lock. You'll find a Toluca Postcard inside, and examining it in your inventory triggers the Stillness ending.
