A few more I've come across that aren't yet included, no doubt many others will have also done so too, but anyway:
Mall: Long/Short Rail: That downhill section just before the silver structure thing (you know that planter with the two beams in an "X" shape). When you're at the top of hill, grind the rail on the far left/right and continue to end of the grind, forgot which way round was which but both should pop.
Skatepark: Across the park: Like across the gym, simply manual from one side of the room to another, widthways I think so towards the judges booth.
Hanger: Lil' light hopper - use the halfpipe to air up to the light shafts, then grind a long towards the helicopter area and hop accordingly where there's gaps.
Light Corner - from the above, when you're in heli area and the light shafts turn sharply to the right, jump and try and continue the grind on there.
Instrumental landing - I got this by accident, jumped off the plane wing, and landed in manual, so maybe that's it.
High' Stepping - there's a red rail at the back on the half-pipe side of the room, I think I lipped it to get that.
Wing to rail - remember this being awkward, but you have to launch from that plane and land in a grind on the mid rail, difficult to get because of how awkward that wing is.
Marseille:
Steppin Out - where the up, up, and away gap is (really thin QP), you need to manual that narrow strip above (including a trick jump and back into manual where there's a step) took me a few goes, but should pop.
Le Dumpster Saut - go to the circular mini-pool (has a red bull logo on the floor) air out at an angle going left, and land in a grind on the red bin (the one you get another gap for lipping).
Le Rail Saut - now go to the mini-pool where the above mentioned red bin is, like above air out and land in a grind on that yellow-semi-circle rail that goes around the pool with the red bull logo in the middle.
Trashman gap - find the long red bin-train (3 in a row), jump on it and manual (it's picky about manualling the whole thing, so need to make sure you land in a manual right at the end for it to register. NY:
One Side - go to top of subway elevator, grind one handrail, and hop to the otherside
Downhill jam - manual all the way down one of the slopes that lead from the road to the low area (where the Joey statue is). Left or right side, doesn't matter.
For Downtown: Mechanic gap: where that truck is halfway out the garage, perform a wall ride up and over it.
Box office gap: Manual from back of cinema, all the way out and down the path until you reach the street
Skyway gap: manual from roof through the secret glass tunnel
Super skyway gap: as above, but keep going along the whole corridor until you reach the end of the corridor and can see that cramped truck that you usually enter from
